Paying for your doula: OHP, HSA & FSA
Doula support is covered by the Oregon Health Plan, and you may be able to use an HSA or FSA. Here is how each one works, and how I can help you sort it out.
Yes, I take the Oregon Health Plan (OHP)
I am a birth doula certified as a Traditional Health Worker (THW) through Oregon Health Authority, which means I can bill OHP.
If you are an OHP member, your doula care is covered. There is no bill for you, and no reduced-rate paperwork to sort out. We just get to focus on your birth.
What OHP covers
- Two prenatal visits before your baby arrives
- Continuous doula support on the day you give birth
- Two postpartum visits after your baby arrives
- Up to four additional visits, prenatal or postpartum in any combination, for twelve months after the end of your pregnancy
Most OHP members are enrolled in a Coordinated Care Organization. CCOs cover the same doula benefit, but each one handles enrollment a little differently. Tell me who your CCO is on our discovery call and I will take it from there.
Using an HSA or FSA
If you have private insurance with a health savings account or flexible spending account, doula support is often an eligible expense. It usually is not automatic, though, and the rules vary by plan.
How to find out
- Call the number on your HSA or FSA card and ask whether doula or childbirth support services are eligible under your plan.
- If they ask for one, request a Letter of Medical Necessity from your OB, midwife, or primary care provider. This is the step that most often unlocks coverage.
- Ask me for an itemized receipt. I will include the dates of service and a description of the care provided, which is what most administrators want to see.
- Keep the receipt with your tax records in case your plan asks for documentation later.
I cannot promise your plan will reimburse you, and I am not able to bill private insurance directly. What I can do is give you clean paperwork and answer questions from your administrator.
Common questions
Does OHP really cover a doula in Oregon?
Yes. Oregon added birth doula services as a covered benefit for Oregon Health Plan members, and doulas who are certified Traditional Health Workers can bill for that care. Oregon was one of the first states in the country to do it.
Do I need a referral from my doctor or midwife?
No. You can reach out to me directly. If your plan or CCO asks for anything extra, I will help you track it down.
What if I am not on OHP and do not have an HSA or FSA?

Do you offer sliding scale pricing for private pay?
Yes. I also offer sliding scale pricing. If we chat and my regular private pay price is out of your budget but you really feel like I am the best fit for you, tell me.
Do you serve clients outside Portland?
I serve Portland and the surrounding metro area. If you are a bit farther out, ask me on our call and we will figure out whether it works.
